Service offered in order to enable participants to gain access to services, activities and resources, as specified by the Service Plan. This service is offered in addition to medical transportation required under 42 CFR §431.53 and transportation services under the State Plan, defined at 42 CFR §440.170(a) (if applicable), and does not replace them. Whenever possible, family, neighbors, friends, or community agencies which can provide this service without charge are utilized.
Counties Serviced
Metro East Community Care, LLC Services Provider Agency is a New Jersey Department of Human Services, Division of Developmental Disabilities (DDD) approved Service Provider Agency authorized to provide the above aforementioned direct care services to individuals with Developmental and/or Intellectual Disabilities residing in municipalities located in all 21 Counties within the State of New Jersey. Such Counties serviced by Metro East Community Care, LLC are as follows:
